1. 進行服務簡介 (約5-10分鐘)
2. 探訪關懷一群居於屯門區，近期出院的長者(2-3戶) (約1個小時)
3. 回集合點，向社工匯報探訪後期望跟進的事項 (約15-20分鐘)
整個活動時間預計1小時30分鐘
探訪活動將分別舉行2次，參加者可只參加其中一日活動，感謝大家的參與
